WellTrackONE helps physicians provide the Annual Wellness Visit to all of their Medicare patients. Our end-to-end system provides the service to contact all patients, schedule them and gather preliminary clinical information. We then provide the nurse to administer the wellness visit using our proprietary software. Our software produces the Medicare-approved reports that are integrated directly into the physician's EHR.
