We provides post-discharge services to high risk populations. Tribus Transitions also works with high cost and utilization patients that have failed with traditional care management. We accept assignment of such patients and oversee all community activities to include in home medication reconciliation, patient and family education, information tracking, referral follow up, and family/caregiver support.
