Company Summary
Positioning Statement
The SeeYouNow web-platform is a communication channel between patients and their clinical care team that allows patients to record their symptoms using two conventional assessment techniques; the Edmonton Symptom Assessment System (ESAS) and the Patient Reported Functional Status (PRFS) assessment. This application will reduce trips to acute care by enabling clinicians to see trends and act on them proactively, allowing clinicians to better meet patient’s palliative and chronic care goals.
What is your product/service?
A communication channel between patients and clinicians and between clinicians in the palliative and chronic care space.
What unmet need does the product/service fulfill, or problems does it solve?
Current best practice is to complete symptom assessments during home care visits which are sporadic. SeeYouNow allows patients or caregivers to complete assessments as often as proscribed by their clinician. This leads to a better quality of care for the patient, more efficient use of clinician’s time, and fewer trips to acute care.
Who is your typical customer that has the above need?
SeeYouNow has three different customers: the patient, the clinician, and the medical system.
Who are your competitors – or how do people solve the problem currently?
Currently symptoms are tracked by completing a paper copy of the symptom assessments and comparing it to a previous hard copy. In Ontario there is the ISAAC system at Cancer Care Ontario care centres, which is a kiosk that can only be used when a patient comes in for visits.
Why is your product or service better than the competition or alternative way to solve the problem?
Using the Edmonton Symptom Assessment Scale and Patient Reported Functional Status tool, the SeeYouNow platform securely collects patient information. Their clinician can now view these assessments with associated comments and see trends based on historical data from the patient. Trends can be compared to past cases and proactive preventative measures can be taken.
