Matt provides the vision for PrimeWellness from his experience as a physical therapist in both the outpatient and home health setting. He earned his Masters of Physical Therapy (MPT) from Georgia State University in 2006. He is a recipient of the Georgia State Division of Physical Therapy's Gordon Seagrave Cummings Scholarship, awarded to individuals who promote self-learning, critical thinking and competence.

Blake focuses on implementing the PrimeWellness vision. He is an Innovation Coordinator with the Veterans Affairs Center for Innovation, where he manages pilot implementations of new models of care and other cutting edge solutions for the Veterans Health Administration. Blake's work on PrimeWellness is heavily influenced by his exposure to emerging telehealth care delivery models at the VA.
