We are web-based care coordination service. Our focus is to save healthcare professionals, families and seniors time when they're in need of a care service. Our system allows healthcare professionals and individuals searching for care to enter patient information and find an available care provider or vacancy. Once a client transitions in the care of one of our care partners they pay a referral fee for that closed lead - avg fee ($2500-$5000).
