CareSmarts is a patient engagement platform that leverages two-way text messaging to engage and motivate patients in chronic disease self-management. The platform utilizes an alert system to prompt care intervention, significantly enhancing patient engagement. In a clinical study at the University of Chicago, recently published in Health Affairs, results showed reduced HbA1c, improved med adherence and significant cost savings to health plan.
