We provide a team based health care delivery model that provides comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes. We focus on increasing health literacy and ensuring patients stay focused on their health care goals. We re-purpose veterans to be competent patient navigators helping patients and their caregivers in the continuum of care.We will significantly reduce health care costs.
