Afoundria has developed a mobile, cloud-based electronic health record (EHR) for use by providers of post–acute care (PAC). PAC constitutes 18% of health care spending AND is responsible for more than 70% of the variation in spending across populations. Implementation of our application has demonstrated that our technology and services will increase the revenue and productivity of a practice while concurrently improving care and reducing costs.
